HCPCS Procedure & Supply Codes - V2410/2430/2499

Need some help:

For those who familiar with these codes: Can these codes be used for any "Aspherical" lens or it has to be some specialty aspherical lenses (if so, what are they ???). And, would V2499 be used for TF and PAL lenses?

V2430 - VARIABLE ASPHERICITY LENS, BIFOCAL, FULL FIELD, GLASS OR PLASTIC, PER LENS
V2499 - VARIABLE SPHERICITY LENS, OTHER TYPE

That's an "add-on" code for asphericity, so you would use the normal code for the standard price and then use the 2410/2430/2499 code for the additional cost of the aspheric "upgrade".
