    Suggestions for a "TAP" lens (thin as possiable)

    Here is the requested Rx. Notice I said "requested".

    OD -1.50 +2.25 X 019 2.50 ADD 18 Diopter BO (i dont see that happening)
    OS -3.00 +0.50 X 023 2.50 ADD 2 Diopter BD

    Patient wants to use transitions and a 7/28 trifocal. PD is 59/56

    The frame is A-49 Grooved semi rimless frame that the pt has been useing for years.
    DBL-18
    B - 35
    ED - 50

    Im speaking of a patient that will be 80 years old in the spring and for her age really seems to have her faculties about her and is rather mobile.

    With todays technology and materials what can I do to give her the best looking lens? The following is what I did for her two years ago.

    Useing that same frame the requested Rx was

    OD -1.75 +1.75 013 3.00 ADD 5 Diopters BO
    OS -4.75 +0.50 014 3.00 ADD 10 Diopters BO and 1 Diopter BD

    We used 7/28 transitions and polycarbonate LFG lenses and we did get the 5 BO OD and OS ended up being 8 BO and 1 DN.
    She is sensitive to base curve changes. I had a remake last year on account of that and I want to keep the base as close to a 400 or 425 as I can.

    So any input from out there before I start this job at the lab?

    You need to find a lens blank with a thick edge!

    18^ BO isn't hard, you just need to be smarter than the lens As Chevypow said, use an extra thick blank like those from X-cel.

    The concern I have is getting a photochromic blank in the EXTRA thick it will need to be. Maybe I just don't have the right resources when trying to find one the other day....

    If you're set on a photochromic trifocal, I'm not sure that you really have a lot of options.

    I would distribute more of the prism to the right eye, however, since this will better balance the cosmetics relative to the higher minus left eye and reduce the blank thickness needed for the left lens. (It looks like you did just the opposite the last time.)
