When one changes the pantoscopic angle one also changes the cylinder abit, sometimes a very minute amount. (You can check this in your lensomer (manuel ones at least). Tilt even a plano enough and you will see cylinder eventually. One also is changing the position of the optical center. Thirdly in a polarized lens one is changing the incidence angle of reflected glare. Any of the above might have something to do with this.
The problem is, changing panto in a Wayfarer can be a problem.

Raj: The person might also be raising the DOC a few mm, by increasing the pantoscopic angle, in situ.

Could the Blurry be from the Poly?

Are you sure the patient doesn't have an add in a progressive that you didn't tell us about?

Do you have the old set or Rx to compare to?

    Change in Rx?
    Change in material?
    Change in measurements?

You'll probably find something in one of those three. (I'll assume the frame sits the same as it has before)

    Most likely problem

How does the optical center align with as worn pupillary location? Especially vertical center.
While powers and cylinder are not that high, this probably be the problem. Suspect this is even more probable in poly than other materials.
