    If you have a good image copy, see if you can access the EXIF data on the image. That will usually include the person or company who that image belongs to, if you are lucky there will be an address or url you can go to for contact info. Whoever created that image most likely markets that monitor stand.

    If you are a little handy, you can buy an old keratometer mount for just about nothing. You could then attach the monitor to a mount plate and attach that to the k-mount, using slip on or screw on pipe that wont show. You will save a lot of money. Also, run those wires thru the hollow in the monut, so you'll get a real clean look.
    Another way is to mount a slightly larget shelf and mount the monitor on that shelf, Zeiss has a mount that comes with their retinal cam which attahes aa monitor
    to the shelf and lets it spin. That would also free up that other arm
