visual acuity

hi there, first off may i say that i no know very little about the optical industry but rather an engineering industry. however i was filling in a medical form for my work and was presented with these on the form:

left eye uncorrected visual acuity
left eye corrected visual acuity

right eye uncorrected visual acuity
right eye corrected visual acuity

first question, what is this ?

second question,

i went to my optician to try to get the correct information and he wrote this down a piece of paper.

V.A left - 6/5
V.A right - 6/5

so what do i right on my form ?

is it possible for anyone to redirect me to somewhere that this question could be answered. its just that i have posted this question on other website that i have got no answer, i left that posting on a professional website that i may be bale to get an answer. however i do understand that guild lines and do understand your opinion :D. thank you anyway

I would suggest you take your form to your optician and let him fill out that part for you.
