    A lens of similar type (matching bifocal in the case of lined bifocals) or just single vision, without a specific foci, that is close enough in power to "balance" the appearance of the other lens. Years ago I saw it written out on an Rx."Balance for appearance".

    I believe this would include balancing for weight too. You can also use a single vision lens to balance a PAL.

    I have questions about the balance lenses too:
    1. Can the balance lens use any diopter/power just its appearance/weight matches the other eye?
    2. how to define the eye need a balance lens?

    1) Yes. It is cosmetic to match minification (Myope) or magnification (hyperope) of the eye and orbit behind the lens. I almost always will make it a spherical equivalent if the other eye is astigmatic. If I have any doubt I would ask the prescriber and/or read the power from the existing glasses.

    2) Maybe an OD will correct me if there are any exceptions, but I can't recall a time a Doc asked me to use a balance lens where there was any usable vision in that eye. In other words the patient is effectively blind in that eye.

    Another thing to note- when you are presented with a prescription indicating a balance lens, you always want to make sure you are fitting the patient with an impact resistant lens material, so as to protect their good eye.
