The only places I can think of are X-cel and Vision-ease or maybe Younger.
You can go with a digital proccessed round 35 or 45!
I do know a company in Belgium that can make you this. I believe they export to USA if you want them to (don't know the cost though)

Does the written Rx provide any kind of prism for near. At a 20cm working distance, there would be a significant amount of vergence demand. What is also possible that if this Rx isn't really meant for walking around, you could ask the doctor to reconsider writing the Rx as +2.00 in the major lens and +3.00 add. I do this often if I think the patient isn't trying to watch television but likes just a low vision add. I do this to simplify supply of the lens and to lessen the impact on the patient's wallet or pocketbook.
Doesn't that mean somebody who uses only 1 eye? Vergence is much less of a problem then, since you can use an inset of zero in the lens. Also, Finefocus doesn't specify if there is an Rx for distance or not.
On a sidenote, you could always go for a good old Franklin Bifocal (cut 2 lenses in half, and glue the halves together)
Adds over +4.00 are pretty much limited to cr39 round, FT25 and 28. Younger might still make a cr39 FT35 up to +6.00. A +3.50 Add for general purpose, and a separate pair of SV readers are another solution.

Thanks for the info and ideas. He is monocular, distance Rx OS only -1.50 -1.75 x 116. Patient is an MD, uses them for seeing patients (plus general use), must be able to walk around.

It's even better, I think you can ask the prescriber to under Rx the distance lens by +1.00. This gives about 20/40 vision and even preserves intermediate vision when the patient is looking up. Use a +4.00 in the add then. The MD doesn't need 20/20 vision walking around the clinic, even if monocular.
This can be done by Ice-Tech with free-form in a dual zone. Alan is at Mido, but I am rather confident he can surface that for you in any material.
