A lot of lens vendors make a 1.67 Transitions that you can A/R.

If the patient wants the best, then I would say the Shamir Autograph II (You didn't list what the seg ht is, but I'm guessing it's higher that 11 :p).

Also available in 1.67 Transitions in non-free form (in no particular order):

    Essilor Accolade
    Shamir Creation
Zeiss GT2 (or GT2 Short depending on seg ht)
    Younger Image
    Essilor Ovation
Seiko Proceed II (or III depending on seg ht)
Fear leads to anger, anger leads to hate, hate leads to chucking a lensometer across the lab.

    me.

Try Varilux Physio or Comfort (360 if you like) for an RX like this.
I'd make them poly or 1.6 index (1.67 unecessary).

    Don;t forget Avance.

I would fit him in a 1.60 short progressive with a smaller b and shoot for a seg of 16mm.

He has 3 diopters of power imbalance which will affect the vision in the distance as well as near but most likely both depending on the prism thinning.

Fit is more important then design in this case. and probably 90% of other cases but who's counting :p
