    Holy Moly!

    I'm just making a set of lenses for an old customer. She has had cataract surgery done by a pretty new surgeon near here. She is going to have a terrible time adjusting to her new glasses, I fear.

    Old Rx:

    OD +5.00-.75 X 81
    OS +4.75-.50 X 73
    add +2.50

    New Rx:

    OD +.25 -1.25 X 97 OK so far...
    OS -4.25 -1.00 X 170 WOW!
    add +2.75


    Am I wrong in thinking that is a pretty poor result OS?


    I just also made a right lens for a new customer that had surgery in another state. Her Rx was around -2.25 with some cyl. Replacing OD in a Silhouette drillmount. She was unhappy with the thickness of her new lens and I assured her that they were right and thinner than her old lens.She thought they would be very thin after surgery. Turns out, she was formerly a -6.75 in that eye, but the surgeon didn't correct her any better than that because it now matched her current OS lens. Nevermind that when she gets her other one done, he'll have to leave her at -2.25. He wanted to reduce the potential imbalance between surgeries, but now she'll not have a very good resuly post-surgery. One eye will be better, but one will not be any better after surgery.
    To me this also doesn't seem quite right or within what we normally get post-op.

    Super mono-vision? A +16.00, +20.00 implant, whats the big deal.;) oops!

    I hate when the Doc's take hyperopes to either a monovision, or take them to slightly myopic, as they hate it. Leave them a tad on the plus side OU, and they will be fine. JMHO

    Being a post op catarct myself with an Rx just for reading on both eyes I believe that the patient got a wrong implant inserted into th e left eye.
    Can be corrected by re-doing it again. Bothersome situation.

    Looks like surgery on the right eye only. Are you sure he did both eyes?

    Before we rant against the doc we should remember there may be other issues we're not privy too involved.

    I would request the medical chart. I cannot tell you how many times someone has changed a plus to a minus at some point... In one practice I was in with a super high remake % over 12% of the RX's came to the Optical with a clerical error.

    It would be unusual for surgery to be done on both eyes at the same time. Docs here prefer to do one, then the other after the first eye has recovered fully. Also, the actual label from the implant should be in the chart letting you know exactly which one was used.

    Could be just a clerical error.

    Had a patient come in a couple of weeks ago with 5D BI OU prism (never had prism before) called the MD to verify, and sure nuff, the Dr. said 5D. After they were ordered and didn't work asked for the chart and it should have been .5 D.

    You want a really unhappy patient wait 'til a myope ends up even a little bit hyperoptic. Doesn't matter whether refractive surgery or catarac, dey gonna be PO'd.

    That's a lot of cyl to change as well. 1.5D, 90-ish degrees off. I bet there were complications.

    At our location, we do quite a bit of post cataract Rx's and I've been getting some strange post surg Rx's lately too. They don't all see the same Dr. either. Maybe there's some screwups at the implant factory, who knows?!? Imagine this TV commercial in 5 years, "if you were a victim of the ________ cataract implant lens, call us at 1-800-Lawyers and we'll evaluate your case for free!!"
    I don't claim to understand everything about cataract surgery, but I wish Dr's would talk to patients about their Rx changes (especially post surg) that way we don't have to explain all this. Sometimes I can tell that a pt is skeptical about what I'm saying and they say "my Dr. didn't mention anything about that" I'm thinking to myself, the Dr. is not the one who did your refraction either.
