Prescription help

Hi. I'm trying to order glasses online for my mom, but don't know how to enter her prescription. Here is a scan of the prescription:

She needs one for reading and one for the computer.

I wonder which 'other eyecare related field' it is that doesn't know how to enter an Rx. I don't think the back side of the phoroptor counts as an eyecare related field.
