Medicare proof of delivery

Does anyone have a Medicare proof of delivery? Or can anyone tell me where to find one?

I have the forms from DMERC, Is it the same for all states? Yoy can go to MEDICARENHIC. COM for more info, or give me your fax 3, I will fax you what I have from Ma.

Is there any way I could get a link to where the form is? We use the ABN, and the latest webinar from nhic just said to have the patient sign a document that invoices the product you're dispensing. I'm interested if there's something else out there!

The latest course I took from Allergan Access stated a signature on the order form dated at the time of dispensing would suffice. They even provided a sample of a reccommended form.

I was really under the impression that the ABN form was all that was needed of course the last time I went to a class was 2 years ago. Alot could have changed since then. The Pt. signs both sides of the ABN form.
If this has changed, Please someone send me the glasses release form if there is one.

OK cool. I have the same info from the webinar for NHIC. I couldn't find any "official" form for actual glasses dispensing, just pretty much what you say.
