I have a patient who is moving to Salt Lake City, UT. She has just purchased 2 pair of glasses and we will ship them to her. I need to find a certified/licensed optician in SLC preferably in or near Temple Square.

SLO moved a while back, I think they're on something like 30th South and W. Temple now.
