    Don't think with 20, you will have a choice. Do 10 out, and 10 out. Good luck with that.

    I've done 20D (actually have done almost 30D) and it can be done, just need to be careful. I wouldn't split unless absolutely nessesary.

    Most diplopia is binocular (if this is the reason for the prism) and may be split.

    press-on prism

    any reason why this hasn't been brought up? Even if you split the the prism there is gonna be a thick edge. The power is almost 0 on the 180.

    If the vision is much worse in the left eye by spliting the prism you create a wors optics in the right (better eye). If thats the case patient is going to be much much better with as much prism as possible in the worst eye!

    Follow the Dr's directions very carefully and DON"T split the prism, you will only make things worse if you do.

    Man I tell you if you can get 20D of prism in that left eye send me a picture. The Rx along the 180 in that eye is -0.16D so it would need to be decentered 1250mm good luck finding that blank.

    My late neighbor had a similar stroke ten years ago. His right eye turned in and we started him at 6D prism, ground in. When it got to 15D, we started with the press on prisms until we maxxed out at 40D, still not enough. No doctor here could help, so he went to Ohio State and had successful surgery, which only lasted for a couple of years until the prism crept back in.
