    CL - Forms - pt handouts

    Hi everyone -

    was wondering if anyone has some contact lens dispense sheets - that you hand out to your pts -

    Our office is currently not using any - and I feel that by offerign a hand out - after an I&R - would only help those new to wearing contacts comply with say wearing schedules - care - and follow up - as well as the actual Insertion and removal of CL's

    I have looked around on the net - but with little success - but

    I would really appreciate your help !!

    Patterson-colwell has some that I bought a few years back. They are duplicate, one goes to patient, one in the record.

    I faced the same issue at the last LC I worked at, as we also did pre testing, Cl training and the like, but no handout. I would literally handwrite bullet points for the patient to follow and some tricks and tips to handling Contacts. I say do the same on letter head, have the patient sign a copy and place in records to show they were properly told how to handle and care for contacts. Obviously they would get a copy as well.

    I find making and typing up your own easier as it can be customized to your practice, and extremely inexpensive thanks to Kinkos.

    I used Excel and Word to create them, FYI. Simple, yet effective.

    I've always wanted to design our own dispense sheet. Unfortunately, there just isn't enough time with everything else in the day. For now, I use the dispensing sheets from CL solution companies.

    For our GP dispenses, Boston makes a really great brochure. I'll have to find the title at work tomorrow. All I can remember right now is that it's dark blue...

    There isn't always enough time during the day but taking an hour before or after work would get it done. Using someone else's template may make it easier but ultimately you want to design your own to your own specifics. Don't forget to include a signature line for patients or their parent/guardian.
