    Eye Exam Forms

    We are in the process of updating our exam forms. The idea is to make sure 1)all the steps to a Comprehensive exam are covered in its unique section 2) a good layout to use for use on computer (we are converting to My Vision Express)

    Our current cards has places of shared information, such as history, pressure tracking, cupping,etc.. and up to 3 years worth of individual rx/refraction information. We maintain contact lens information on special envelopes we keep their charts in so we can keep vials, trials, invoices etc in and records of disp/sell. Doctor H is a stickler on contact lens compliance..The forms are way out of date though, and the design is not valid to today's standards..or at least doesn't flow well.

    So do any of you have a form you wouldn't mind sharing so we can garner ideas for how we would like to arrange/create our form? I have copies of the VSP recommended form, and I have received a few from some of the medical companies, but still haven't seen one that seems to fit the bill.

    Well, if you are converting to My Vision Express or any other practice management application you paper form should, at minimum, duplicate exactly the schema of the software.

    This particular PM software has you scan in your form, then you can use the computer to fill it in..
