    Need Help with PAL Choice

    Hi everyone

    I am hoping to tap the wealth of knowledge - and also everyones experiences -
    I have a potential problem PT - he is a surgeon and also wants to be a 1st time PAL wearer

    He had told me he is "" looking for the best vision - not To concerned with weight or thickness - but wants the best optics ""
    Well - as you know - most Md.'s are in the same group as my engineer PTS so my ?'s are

    ? YOU SUGGESTION ON A FREE FORM PAL ( PERF. NOT ESSILOR PRODUCT ) AND ALSO A STANDARD PAL -
    I was reading my lens bible - and see Ziess seemed to have allot to offer - and one in particular was aval. with a near zone advantages -n ( which i thought would be great since this is what he would be doing at work - also this lens was aval. in a 1.60 and 1.67 so therefore i thought i might get better optics in the 1.60 ?

    If you could chime in - and tell me your best design choice to try in a free form - and also standard pal - and why - I WOULD GREATLY APPRECIATE IT - can anyone tell
    I got a v-mail saying he will be coming back to see me today - to discuss his options - "" SOUNDS LIKE A FUN easy afternoon "" -NOT- LOL

    i did recom. actually 3 prs for him -

    the pal - for daily activities -
    a occupational lens - such as access - or similar - to give him wider areas - then the pals -

    that he may use for his
    computer and reading and also can maybe use in OR - since everything is in arms length -

    and a pr of sun -

    i did want him to 1st try the pals - since he is a good chance to have rx issues - and may have to go back to recheck his - rx - just seems like that kind of pt - also that he may even need differant refractions for his regualr glasses - and then his occupational needs - so that was my plan - to get everything picked out - and hold off on all but the pals - till i feel better about his new rx -

    price is no real huge concern for him - i think -
    however i have found it may be difficult to realize it is time for pal - and more imptly - that the vision will never be the same as it was 20 yrs ago LOL

    PAL help!

    The Zeiss Gradal Top is a very good PAL. I have used it with good success with my pt's who are also drs. You did not mention the seg height, if the seg is 16 or 17mm I would go with the Shamir Piccolo. I have used both these PALs on my own cousin who is an MD. He really likes the Zeiss lens best but opted for a smaller frame this year. For a basic PAL with good results and a good price point try the Younger Image.:D

    This guy is begging for multiple pairs. Office, Near, and a PAL.

    Any PAL is a compromise, especially at his add power. Sure, give him a high quality, latest generation PAL for general use. But if you try to give someone this picky, with that much add, the "best optics", you would not be doing him any favors by recommending a PAL for surgical procedures and paperwork.

    Chris R will use this quote against me forever, but dare I say FT for long hours of paperwork or surgical procedures. I had a radiologist once that suggested to me that his +5.00 Rx, with 2.50 add be made in an Exec with the top set for a 24" working distance, in a 56 eye aviator of course. Even though the lenses are over 1cm thick, he loves them. I shouldn't have let this guy prescribe his own "office lens"; I should have anticipated the needs of his job and suggested a solution. That was a lesson for me.

    I had some surprising luck converting FT wearers with a GT2. I am guessing that he wore a FT before since his add is 2.25.

    As far as material - Trivex is the way to go. Even though he says that weight and thickness don't matter - you can explain that the optics are unnoticeably different than plastic or glass, but half the weight and 30% thinner (due to 1.0mm CT possible). Although I'm sure a GT2 isn't available in trivex; an Autograph or Creation may be.

    This Rx not recommended for semi-finished PALs , but if you would like to try ,may be the Nikon Presio i15 is will be one of the best choice and Nikon Presio i13 for small frame.

    My openion is more than Rx SPH. - 7.00D is not suitable anymore for semi-finished PALs.

    Please try to going for any free form PALs.

    All of the most bad free form PALs in the world will be total better than Varilux Physio in your Rx. If somebody not agree with me , you can try and you will see. :cheers:

    Many PALs company try to lies us that , their semi-finished PALs is better than every free form PALs , may be they are little right in Rx Plano ADD 1.00D.

    The problem is only few wearer have Rx Plano ADD 1.00D.

    If it's a PAL you want then I recommend the Definity or Physio but a lot of times I put someone like this in a wide seg trifocal. 8x35. I have two dentists and an orthopedic surgeon in them and they flunked out of PALs. Not enough clarity and precision. Call me a dinosaur.

    I only have dispensed two of these pair thus far as its new to me but Kodak's Uniques lens seems pretty fantastic. It has generated excitement and I can update you as two other pair came in yesterday, awaiting pt delivery. In any case iti s a free form lens and I think worth you looking into. I know you mentioned you dont prefer essilor however the physio is a nice lens also, I however would discourage definity. I am not a fan.

    I must also agree this guy is screaming multiple pairs. Impress him with your knowledge and fill all of his needs and you shouldnt have to see him for a while.

    I've heard fantastic things about the Shamir Autograph! I agree with above posts concerning multiple pairs. I also think impact resistance may be a concern, so I would discuss that with him as well. Plastic should be fine, as trivex is so limited in lens designs.

    ? on reading suggestion

    just was wondering -

    would you not feel - by filling a occupational - intermediate/reading lens such as a access or office lens -

    that this will provide plenty of reading room - so that he will not have to those seperate NVO glasses - you suggest ?

    i had thought that using the access - ect would compensate and actually be better for him - then NVO ?

    as for the suggestion of the trifocals - that was going to be one of my last options - since i like to at least try the new tech. 1st - esp since he really has no bad habbits - or expectations of the pal -- yet LOL

    My own Rx is very close to your patient (the surgeon) and I personally switched myself from Physio (which I loved) to the freeform Kodak Unique which I find marginally better. Everything about freeform that you've heard is true. The optics are sharper, wider, better dim light vision and no side distortion. This does not mean that your doctor might not find that, for surgery, a special pair of lined bifocals might work better. Telescopic magnifiers are often used by surgeons on top of their glasses. What kind of surgery and head position does he find himself in? If one pair of glasses is going to be worn, I would recommend a freeform hi-index
