    Confused What is your choice?

    Hi all.

One patient visted my office to buy his first PAL. He is 45-years old. Although he has no medical anomalies, shows some different refractive error. his prescription is as below.

    OD: PL Add +1.25
    OS: +0.25 -2.00 180 Add +1.25

As you know well, if he wears his PAL, he may feels vertical prismatic deviation as well as it tends to raises patient non-adapation. in addition, the power of near portions are +1.25(OD) and -0.50(OS) respectively. therefore, it becomes antimetropia.

I'm under considering to order Zeiss HS with slab-off to remove vertical prism effect.

If you have another idea, please suggest.

    it is very urgent.

With a OS -2.0 cylinder, he was already wearing glasses before? Or not?
    So there already was anisometropia!?

    Isnīt the *average* OS near power 0.25+1.25 - (2.00/2) = +0.50!?

    hum...

    oh~~ I made a mistake.

xiaowei! you were right regading the near portion spherical equivalent power. however, the prismatic imbalance still remained on the vertical meridian(90).

in this case, are general PALs appropreate for the patient?

If the astigmatism is at axis 180 in minus format, the whole cylinder is in affect on the 90 meridian. Therefore, the reading power in that lens would be about -0.50 (-1.75 in distance, plus the add)??
Is that right?? (I've only just begun to learn how to do those calculations so if I'm wrong, please correct me!)
Or, are you calculating it at 45 degrees away from the 180 due to inset?
