We just bought the contact lens portion of the business from our O.D. and now we need to develop a form for contact lens wearers for recording orders, findings, etc.. Hoping for something that would fit on a 4x6 card. Does anyone have anything already printed up that we could take a look at?

Our form is gear towards the pt. it lists:
-wearing schedule
-expectations
-type of cl
-type of solution
-potential problems associated w/ cl
-amount of deposit for trials (this saved our practice from 1-800)
-signature of pt. & guardian if minor
The pt initials every section and keeps a cop[y for themselves. We keep a copy, and it has gotten us out of more than a few jams w/ non-compliant pts.
