Apart from the obvious Franklin Split, does anyone know of a flat top bifocal with a negative add, ie top ½ of lens approx +5.00, bifocal seg of +4.00 power thus an add of-1.00

I have contemplated how to do this in my younger days. Never seen one or seen anyone who had a condition requireing this. If you learn whatever indicates (from an pathology standpoint) this please let me know.
First I would talk with her doctor and see if he wanted a plus add (total +9.00) before I wasted my time proceeding further. Next I would see if there are any bifocals what could be mounted upside down that would have a desirable outcome.
Lastly I have never been offended by a Franklin bifocal, what do they do that's obnoxious? The effect with same should be the same as an executive without the added prism and centers can be set as needed.

How about just surfacing and edging them upside down using +1.00 add and grinding in +4.00.

I did one pair like that years ago, used an exec turned upside down.
The only thing I can think of to treat with a minus add is a patient with convergence insufficiency. I know it can be "taboo" but there are some very simple home vision therapy techniques that you could recommend. I just made up a simple instruction set and keep around a few brock strings. I know it takes more effort than specs (by the patient) but it works and its cheap for the patient.
Otherwise, until they make molds for minus adds (which will never happen) you are stuck with the upside down executive or NVO.
