Might you consider some very unusual frames that are apparently designed for comfort without going all the way back to the ears?

The Staffan Preutz "SP" frames: These are rimless frames (which I believe are available in both the U.S. and Europe) that do not have conventional temples. They have short temples, about half the length of the usual temple piece, and use a soft pad that clamps directly onto the side of the head, well forward of the ear, via gentle pressure from the springiness of the frame.

    The Staffan Preutz (USA) website:
    http://www.staffanpreutz.com/USA/Preutzsecond.htm

I saw this website when I was doing some wide-ranging Internet research on eyewear, more out of curiosity than for any strictly practical reason.

I'm not representing any product, and I am not involved personally with the optical industry, except as an Rx spectacle lens wearer.

Ain't as pretty as it could be, but it works

Go to Steinmart, get one of the head bands that goes across the top of the head (usually metal core, with leather or some other external decoration) Remove metal core. Weld to temples just in front of ear. Adjust to fit.

Works great. Hide under hair if possible, if not tell patient to wear cap.

    Or....
Look to a frame (NIKE COLUMBIA OAKLEY) that have straight temples, work a bit to fit the frame, the lighter weight the better and be happy that it works. I have a pt whom has been using straight temples for a couple of years and is extrmely satisfied. Also have you considered contacts for the pt?

    A perfect solution by Chip.
You can also just get a regular elastic head band across the top of the head and glasses will stay in place.

Contact lenses could be a option. How about an Adidas type sports frame - some of them have straight sides with silicone end tips that fit by pressing the temples. would have the advantage of being a bit street smart too!

Wouldn't a Silhouette model 7395 or a Nike frame work as well, you have that springy temple tension which I would think would hold the frame level?

agreed, the skull, library type temples would still be prone to slippage.

Yes I have fitted this range. a bit quirky, and odd to wear (puts a fair amount of presure on the head where the side pieces touch), but yes, they do alliviate the need for ears
