Hi all,

My patient , 45, computer user ordered a essilor natural PAL lenses.

Rx:
+0.25
+0.50
add +2.5

fitting high 19
COF plastic frame
first time to wear PAL

when picking up, she complained that
(1) she has trouble to see computer screen clear and her eyes very strain to focus on computer
(2)when reading, she only can see middle part of the sheet but both right and left parts are blurred.

I checked the lenses and PD no problem

Could anyone help me?

Simple solution

Take them out of the PAL and put them in a task specific lens like the Nikon Office, Sola Access, or Essilor Interview.

The client does not need glasses for distance. The task specific lens will allow the person to use the lenses for intermediate and reading.

You could also keep her in an Essilor Natural and use half the add added to her distance and use a +1.25 add. Since she has never worn progressives you could fit her in a FT-28.

Put her full NV Rx is a trial frame an establish her depth of field.

she age 45 WITH ADD250 ?

If she is ok, in my experience may be the hyperope is not enough, therefore the add will be much higher than normal.

However if she really need that reading power, when add increase the field of vision esp. computer segment will be bad.

only way to solve is using office PAL.

To achieve a "soft design" lens Essilor spread the destortion well into the distance. For rxs with weaker spheres and no cylinder sometimes are a problem. Put her in an XL of VIP. Super no line if you want to stay with Essilor.

1. May be over minus or under minus for distance prescribtion
2. The patient may have latent hyperopia
3. May have working distance at 30 cm. if true this case will have
problem in intermediate distance with PALs.
4. The patient may have accommodate problem

IF need PALs you will used PALs with 15 or 17 mm. corridor
and will better if used free form PALs.
OR used hard design PALs with long corridor IF it have in your market.

Not used 11 or 13 mm. corridor or short corridor PALs.

45 year and have add 2.5 D. NOT make sense. Pseudophakia? If not, then I would strongly recommend calling the doctor to confirm and justify the Rx'd Add power.
